Medical Treatment Visit
1. Request form of visit visa.
2. Copy of the visitor's passport provided that it is valid not less than six months.
3. A letter from the Ministry of Public Health confirming acceptance of treatment in a hospital.
4. Copy of the civil id card of the sponsor.
